Vy\u0161et\u0159en\u00ed je koncipov\u00e1no jako partnersk\u00e9 \u2013 p\u0159i vy\u0161et\u0159en\u00ed obou partner\u016f je sou\u010d\u00e1st\u00ed v\u00fdsledku vyhodnocen\u00ed reproduk\u010dn\u00edch rizik dan\u00e9ho p\u00e1ru vydan\u00e9 jako samostatn\u00e1 zpr\u00e1va.<\/p><h3><strong>Informace k\u00a0testu:<\/strong><\/h3><ul><li>Test byl vyroben dle vlastn\u00edho n\u00e1vrhu Genetika Plze\u0148, laborato\u0159\u00ed je pln\u011b validov\u00e1n a\u00a0pro\u00a0pacientsk\u00e1 vy\u0161et\u0159en\u00ed pou\u017e\u00edv\u00e1n od\u00a0roku 2021.<\/li><li class=\" translation-block\">Jedn\u00e1 se&nbsp;o&nbsp;tzv. <strong>\u201eroz\u0161\u00ed\u0159en\u00fd\u201c screening p\u0159ena\u0161e\u010dstv\u00ed<\/strong> (Expanded Carrier Screening) a&nbsp;c\u00edl\u00ed na<strong> nej\u010dast\u011bji se&nbsp;vyskytuj\u00edc\u00ed<\/strong> <strong>v\u00e1\u017en\u00e9 recesivn\u00ed choroby<\/strong> <strong>ve v\u0161ech sv\u011btov\u00fdch etnik\u00e1ch<\/strong>, nap\u0159.: cystickou fibr\u00f3zu, spin\u00e1ln\u00ed muskul\u00e1rn\u00ed atrofii, nesyndromovou hluchotu (s mutac\u00ed genu GJB2), \u03b1 a \u03b2 thallasemie, srpkovitou an\u00e9mii, fenylketonurii, albinismus typu 1 a dal\u0161\u00ed (pro kompletn\u00ed seznam onemocn\u011bn\u00ed a&nbsp;gen\u016f viz <a href=\"https:\/\/www.genetika-plzen.cz\/wp-content\/uploads\/2024\/10\/SE.GP-20-v001-Carrier-screening-vysetrovana-onemocneni.pdf\" target=\"_self\"><strong>Carrier screening \u2013 Seznam vy\u0161et\u0159en\u00fdch onemocn\u011bn\u00ed<\/strong><\/a>.<\/li><li class=\" translation-block\">Celkem je vy\u0161et\u0159eno <strong>103&nbsp;gen\u016f spojovan\u00fdch se&nbsp;128&nbsp;autosom\u00e1ln\u011b recesivn\u011b d\u011bdi\u010dn\u00fdmi a&nbsp;23&nbsp;X-v\u00e1zan\u00fdmi monogenn\u00edmi chorobami<\/strong>.<\/li><li class=\" translation-block\">Design panelu respektuje <strong>doporu\u010den\u00ed mezin\u00e1rodn\u00edch i \u010desk\u00fdch odborn\u00fdch spole\u010dnost\u00ed <\/strong>pro prekoncep\u010dn\u00ed vy\u0161et\u0159en\u00ed (ACMG, ACOG, SLGG \u010cLS JEP) a obsahuje v\u011bt\u0161inu gen\u016f pro onemocn\u011bn\u00ed detekovan\u00e1 v \u010cR v r\u00e1mci novorozeneck\u00e9ho screeningu.<\/li><li class=\" translation-block\">Vy\u0161et\u0159en\u00ed je provedeno nejmodern\u011bj\u0161\u00ed laboratorn\u00ed metodikou \u2013 <strong>sekvenov\u00e1n\u00edm nov\u00e9 generace <\/strong>(NGS), kter\u00e9 jako prvn\u00ed technologie umo\u017e\u0148uje anal\u00fdzu mnoha gen\u016f z\u00e1rove\u0148. U v\u0161ech gen\u016f je vy\u0161et\u0159ena cel\u00e1 k\u00f3duj\u00edc\u00ed sekvence (nikoli jen nej\u010dast\u011bj\u0161\u00ed mutace) a analyzov\u00e1ny jsou i rozs\u00e1hlej\u0161\u00ed (exonov\u00e9) zisky\/ztr\u00e1ty (tzv. Copy Number Variations), \u010d\u00edm\u017e je dosa\u017eeno v\u00fdznamn\u00e9ho zv\u00fd\u0161en\u00ed m\u00edry detekce mutac\u00ed (a\u017e 99%) a tedy i sn\u00ed\u017een\u00ed rezidu\u00e1ln\u00edho rizika p\u0159ena\u0161e\u010dstv\u00ed.<\/li><li class=\" translation-block\">Do&nbsp;testu jsou zahrnuty i&nbsp;<strong>genov\u00e9 varianty, kter\u00e9 mohou souviset s&nbsp;poruchami reprodukce<\/strong>, v\u010detn\u011b vrozen\u00fdch trombofiln\u00edch stav\u016f. V\u00fdsledky t\u00e9to anal\u00fdzy maj\u00ed nejen diagnostick\u00fd v\u00fdznam, ale p\u0159isp\u00edvaj\u00ed i k individualizaci p\u0159\u00edpadn\u00e9 l\u00e9\u010dby neplodnosti metodami asistovan\u00e9 reprodukce (viz <strong><a href=\"https:\/\/www.genetika-plzen.cz\/wp-content\/uploads\/2024\/10\/SE.GP-19-v001-Genove-varianty-souvisejici-s-poruchami-reprodukce.pdf\" target=\"_blank\" rel=\"noopener\">Seznam vy\u0161et\u0159en\u00fdch genov\u00fdch variant<\/a><\/strong>).<\/li><\/ul><h3>Cena a\u00a0zp\u016fsob vy\u017e\u00e1d\u00e1n\u00ed:<\/h3><ul><li class=\" translation-block\"><strong>Pro&nbsp;poji\u0161t\u011bnce \u010desk\u00fdch ZP je vy\u0161et\u0159en\u00ed \u010d\u00e1ste\u010dn\u011b hrazeno ZP s&nbsp;doplatkem 3500,- K\u010d\/osobu. <\/strong>Krom\u011b vyhodnocen\u00ed Carrier Screeningu a reproduk\u010dn\u00edch rizik p\u00e1ru je v\u017edy provedeno i hodnocen\u00ed genov\u00fdch variant souvisej\u00edc\u00edch s poruchami reprodukce. Vy\u0161et\u0159en\u00ed mus\u00ed b\u00fdt <strong>indikov\u00e1no klinick\u00fdm genetikem<\/strong> a&nbsp;navazovat na&nbsp;genetickou konzultaci.<\/li><li class=\" translation-block\"><strong>Pro&nbsp;zahrani\u010dn\u00ed samopl\u00e1tce<\/strong> je stanovena cena testu na&nbsp;<strong>690,- EUR\/osobu pro&nbsp;Carrier Screening <\/strong>(test zahrnuje nav\u00edc vy\u0161et\u0159en\u00ed genu FMR1&nbsp;jinou detek\u010dn\u00ed metodou). Vyhodnocen\u00ed <strong>variant souvisej\u00edc\u00edch s&nbsp;poruchami reprodukce<\/strong> je s&nbsp;doplatkem <strong>60,- EUR\/osobu<\/strong>.<\/li><li><strong>Test je proveden ze vzorku perifern\u00ed krve s odb\u011brem do EDTA.
