Es ist eine Methode mit gro\u00dfem Potenzial f\u00fcr weitere Entwicklung und breite Anwendung bei allen Arten von genetischen Untersuchungen (gezielte Sequenzen\/Gene und das gesamte Genom), einschlie\u00dflich der M\u00f6glichkeit, sie in einem Experiment zu kombinieren. Die Methode NGS (mit sogenannter niedriger Abdeckung) kann auch f\u00fcr das pr\u00e4implantationsgenetische Screening numerischer Chromosomenver\u00e4nderungen (PGS (PGT-A)) oder f\u00fcr das pr\u00e4implantationsgenetische Testen unbalancierter Formen famili\u00e4rer Chromosomenumstrukturierungen (PGT-SR) in Verbindung mit dem Screening numerischer Ver\u00e4nderungen anderer Chromosomen verwendet werden.<\/p>\n\n<p>Wenn die Analyse an einer Probe des Trophoektoderms durchgef\u00fchrt wird, ist es dank der h\u00f6heren Sensitivit\u00e4t der Quantifizierung, des verbesserten dynamischen Bereichs der Ver\u00e4nderungen und der besseren Unterscheidung des Signals vom Hintergrund m\u00f6glich, das Vorhandensein mosaikartiger Ver\u00e4nderungen zuverl\u00e4ssiger zu beurteilen (als bei fr\u00fcher durchgef\u00fchrten Untersuchungen mit Array-komparativer genomischer Hybridisierung (aCGH)). Ein Befund einer Chromosomenaberration in Mosaik bedeutet, dass in der Probe bzw. im Embryo, aus dem die Probe entnommen wurde, eine normale (euploide) Zelllinie und gleichzeitig eine aneuploide Linie vorhanden ist. Diese Ver\u00e4nderungen sind immer mit de novo entstandenen Chromosomenaberrationen verbunden, die im Rahmen der Untersuchung PGS (PGT-A) nachgewiesen werden. In einer Situation, in der kein (verbleibendes) Embryo mit normalem Befund vorhanden ist, kann nach entsprechender genetischer Beratung und nach Unterzeichnung einer informierten Einwilligung der Transfer eines Embryos mit mosaikartiger Aberration in Betracht gezogen werden.<\/p>\n\n<p><strong>Prinzip der NGS-Methode:<\/strong> Aus den entnommenen Zellen wird DNA extrahiert, bei der anschlie\u00dfend eine gesamtheitliche Genomamplifikation (whole genome amplification, WGA) durchgef\u00fchrt wird. Aus erfolgreich amplifizierten Proben werden enzymatisch sogenannte \u201eBibliotheken\u201c erstellt, die f\u00fcr das gleichzeitige \u201eLesen\u201c vieler DNA-Sequenzen geeignet sind. Jede Probe\/Bibliothek hat eine einzigartige Kennzeichnung, die es erlaubt, in einem Experiment die DNA mehrerer Individuen (Embryonen) gleichzeitig zu analysieren. Die Proben\/Bibliotheken werden im gleichen Verh\u00e4ltnis gemischt und die resultierende Bibliothek wird anschlie\u00dfend f\u00fcr das abschlie\u00dfende Sequenzieren vorbereitet. Das eigentliche Lesen der Sequenz (Sequenzierung) erfolgt durch die schrittweise Synthese neuer komplement\u00e4rer DNA-Str\u00e4nge zu den gelesenen Fragmenten. Die gelesenen Sequenzen werden mit spezieller Software mit dem normalen menschlichen Genom verglichen, ihre genomische Position wird bestimmt. Nach der Zuordnung der Sequenzen zu den einzelnen Proben wird eine quantitative Bewertung aller Chromosomen durchgef\u00fchrt.<\/p>\n\n<p><strong>Einschr\u00e4nkungen der NGS-Methode:<\/strong> Die verwendete NGS-Methode mit niedriger Abdeckung (low-pass whole genome sequencing) ist durch die Gr\u00f6\u00dfe der Chromosomenumstrukturierungen begrenzt. Kleine Verluste oder Gewinne von Chromosomen k\u00f6nnen nicht aufgedeckt werden und es k\u00f6nnen gleichzeitig keine weiteren Krankheiten oder Entwicklungsst\u00f6rungen des F\u00f6tus ausgeschlossen werden, die nicht durch eine Ver\u00e4nderung der Anzahl der untersuchten Chromosomen oder ihrer gr\u00f6\u00dferen Teile verursacht werden.<\/p>\n\n<h3>Schema der NGS-Untersuchung
